| 1. Payment is due at the time of your visit, unless satisfactory arrangements have been made in advance with our Patient Account Representative. There will be a 25% discount for services paid in full on the day of service. Co-payments are due at the time of check in for your appointment, no exceptions.
2. Cash or checks will be accepted with a $25.00 fee for NSF checks. For you convenience, we also accept debit/credit and most major credit cards.
3. As a courtesy, we will bill most insurance companies when you assign the benefits to High Desert Medical Center. Your insurance is a contract between you and your insurance carrier. If they do not pay High Desert Medical Center in 45 days, we will look to you for payment. We cannot accept responsibility for negotiating a disputed insurance claim. You will be refunded any overpayment that you make on your account in the event that your insurance eventually pays.
4. Any balance remaining after your health insurance pays, denies or deems non-covered under your plan, will be your responsibility. Payment is due upon receipt of your statement; Statements are mailed to our patients monthly.
5. In the event that you have a private insurance that we do not bill, your account will be considered a self-pay account. As a courtesy, we will provide you with a bill (HCFA-1500 form) so you can bill your own insurance. You are responsible for payment on your account regardless of if your insurance has reimbursed you or not. Private insurance includes, but is not limited to: Motor Vehicle Accidents (MVA), Tricare, Champus and other out-of-state plans.
6. Workers’ Compensation Insurance: before we can treat you for a workers’ compensation claim, you must report your injury to your employer. If you are being seen for an existing claim, you must provide us with your claim number, adjusters name, telephone number and address. We also request that you provide all of your private insurance information. This will allow us to bill them should your claim be denied or if benefits are exhausted.
7. We will look to the adult accompanying a minor, regardless of their relationship, for payment of all services rendered to minor patients less than 18 years of age.
8. In the case there is a suit filed by High Desert Medical Center or Collection Agency for recovery of unpaid balances, you will be responsible for any incurred costs and / or attorney fees associated with this action.
