Insurances we are contracted with:

• Regence Blue Cross Blue Shield
• Providence Health Plans – There are several smaller insurance companies that are contracted through Providence that we expect assignment with. Please refer to the back of your insurance card to see if your insurance is one of them. For example; GEHA and Parr Lumber are contracted through Providence.
• Lifewise
• ODS
• Pacific Source
• Employers Health Alliance
• Cigna
• Medicare
• Central Oregon Individual Health Services (COIHS)
• OMAP

There are many different policies for each insurance. Depending upon the one you have, you may be charged a copay, or a deductible. Please be prepared to pay your copay or deductible at the time of service. Please refer to our Financial Policy for detailed information.

Because each plan and policy are different, it may be to your benefit to call your insurance company and find out what type of benefits they provide for you and your family.

For those patients who have insurances that we are not contracted with, we will bill them as a courtesy to you. However, you will be responsible for the balance that your insurance does not pay, or if they do not respond within 45 days.
